What are Social Determinants of Health (SDOHs)?

According to the World Health Organization (WHO), social determinants of health (SDOHs) are the circumstances in which people are born, grow, live, work, and age. This includes the health system. The circumstances in which a person or community lives are the result of the distribution of money, power and resources, globally, nationally and locally. The policies that are adopted also have a great influence on this. SDOHs make visible the inequities and injustices in which people can live.

What is Sociants?

It is a digital platform that helps identify social needs, and matches them with local community providers to provide resources to both individuals and communities in need. Social needs can be identified to assist individuals and to connect organizations and groups that provide services to them.

What is ReferHelp.com?

This is a space where organizations provide resources through their various community programs and exchange social assistance referrals to address the social determinants that affect the well-being of people in their community, participants and patients.

Who can apply for the help offered by Sociants and the organizations it represents?

Our services can be requested by any person who is a victim of a natural or climatological disaster, who is deprived of their main needs and lacks basic needs.

What type of survey does Sociants use to identify needs?

Our survey of SDOHs is based on the most common social needs people have today, based on findings from the Centers for Medicare & Medicaid Services (CMS). The questions address important determinants that affect each individual, such as sociodemographic information, housing instability, employment conditions, among others.
